Enrollment Guide -  Provides information about the plan and investment options.

401k Plan Forms:
Enrollment Form - Complete this form to enroll in the IUE-CWA 401(k) Plan.

Designation of Beneficiary Form - Complete this form to designate or change your beneficiary for your 401(k) account.

Change Form - Complete when making any changes to your 401(k) account information.

Direct Rollover Form - Complete to roll tax qualified money into the IUE-CWA Plan.

Borrowing or Withdrawing your 401(k) savings from the Plan:
If you wish to take a General Purpose Loan, you must first contact Transamerica at 888-976-8171 or log into your online account and request a loan. This Plan only allows one loan at any given time and must be repaid. Borrowing from your 401(k) retirement savings can lower your lifetime savings amount. When the money is not invested, your savings is not growing. You will also be repaying the loan with AFTER-TAX money.

Primary Resident Loan - Used only when borrowing money for the purchase of your primary residence.

Hardship Withdrawal Form - Must be active, already have an outstanding loan and qualify for the hardship under the IRS rules.

Distribution Form - When you terminate your employment, you can complete this form to withdrawal money from your savings. You do NOT have to take your retirement savings from the Plan when you terminate employment or retire.  

Age 59 1/2 In-service Withdrawal Form - Must be active with an employer participating in the Plan and age 59 1/2 or older.
