How do I change my mailing address
for the IUE-CWA Pension Fund
& 401(k) Plan
  We must receive your change of address in writing, with your signature.  Your employer does not always forward changes of address to our office. 

You can drop us a simple note that has your name, new address, new phone number, new email address, and your employer asking us to change your information in our systems.

If you prefer, you can fill out the appropriate form below.
